Fat grafting of the breast was discussed at the February 2008 Baker Gordon Symposium on Cosmetic Surgery. Cases presented included breast enlargement surgery by fat grafting and reconstructive breast surgery. Breast enlargement by fat grafting takes about 5 to 7 hours, and is labor intensive. A considerable amount of fat needs to be removed from a variety of donor areas to get enough fat to inject under the skin of the breast.

Only about half the volume removed is suitable for injecting. The amount of breast enlargement is limited because too much fat injected may interfere with the blood supply to the new fat, and because some of the fat reabsorbs.

Considerable debate ensued following these presentations.

In 1987 the American Society of Plastic Surgery (ASPS)issued a stong position against this procedure. The ASPS has wisely issued a new strong caution with this. Roxanne Guy, MD, current President of ASPS states:

“Patients considering breast augmentation need to know that fat grafting for this indication is not recommended at this time, unless one is part of an ongoing clinical research study. Even though fat grafting techniques have become more refined and breast cancer detection methods have become more sophisticated with time, creating a great deal of excitement about the potential of fat grafting, we simply need more data before we can recommend that this procedure be generally used to augment breasts.”

This technique is frowned upon because of potential problems and the lack of long-term scientific data regarding this technique. There continues to be the potential problem of interference with mammography. Long-term results are scarce. Of concern is the potential for spreading an established breast cancer as the cannula makes multiple passes through the tissues.

Currently it is felt that fat grafting of the breast is more suitable for breast reconstruction patients, to help smooth out irregular contours following reconstructive efforts.
