One patient writes:&amp;rdquo; I have been diagnosed with vaginismus and was interested in learning more about the botox injections for this. I use dilators currently but still experience pain.&amp;rdquo;</p>
<p>There appears to be a continuum of painful intercourse ranging from mild burning pain with penetration, to the complete inability to have ANY form of penetration including a GYN examination. A number of patients have told us that they had to be sedated to have a GYN exam.</p>
<p>We always recommend that patients try conservative therapy first. For milder cases conservative therapy of vaginismus can be successful when dilators are used in combination with psychotherapy, sexual counseling and physical therapy working with Kegal exercises.</p>
<p>The problem is that in more severe cases, this can take a long time, become frustrating, and hurt relationships. For patients who want to speed up the process, Botox injections done under sedation with a light general anesthetic shortens the entire period to about one month. We have had excellent success using Botox for vaginismus and find that even milder cases of vaginismus are candidates for this approach.</p>

<p>A loss of 60 pounds is considered massive weight loss. &amp;nbsp;It is very natural to have sagging breasts after weight loss as a significant portion of the breast is fat. &amp;nbsp;The older a woman is the more fat she has in her breasts, and thus weight loss will affect the size and shape of her breasts. &amp;nbsp;Depending upon the amount of ptosis&amp;nbsp;(breast sag)&amp;nbsp;a number of different approaches may be used. &amp;nbsp;Women with minimal ptosis may be candidates for augmentation mammoplasty alone. &amp;nbsp;If the degree of ptosis is more severe an augmentation is usually combined with some type of mastopexy, anything from a crescent mastopexy to a full Wise pattern (anchor) mastopexy. &amp;nbsp;A good way to judge for yourself is to gage the level of your nipple. If your nipple is at or above the level of your&amp;nbsp;breast fold then usually implants alone will be sufficient. &amp;nbsp;If the nipple is significantly lower than the fold then a mastopexy is usually required in combination with the augmentation. &amp;nbsp;All of this can be determined at your initial consultation.&amp;nbsp; <br/>

<p><input type="image" height="114" width="117" src="http://www.plasticsurgerypa.com/blog/Image/Answer3.jpg" align="right"/>It certainly sounds like you have lost the majority of your excess body weight and have maintained that weight loss for a significant period of time.&amp;nbsp; The redundant skin that is present will not respond to exercise and therefore surgical correction is the only option to give you the result that you desire and deserve.&amp;nbsp; Certain procedures can be combined, usually based on the length of the procedure.&amp;nbsp; Depending upon the severity of the deformity, certainly a brachioplasty (arm lift)&amp;nbsp;and abdominoplasty (tummy tuck)&amp;nbsp;could be combined. Many people need a circumferential lower body lift rather than an abdominoplasty as this gives the added benefit of a buttock lift and a lateral thigh lift at the same time.&amp;nbsp; A brachioplasty is normally about a 2 hour procedure.&amp;nbsp; It is my experience that patients do much better recovery wise when surgery is limited to approximately 5 hours under general anesthesia. Procedures longer than 5 hours make recovery for the patient far more difficult and increase the incidence of complications.&amp;nbsp; That being said, an abdominoplasty and brachioplasty combined should take approximately 5 hours.&amp;nbsp; <br/>

<p><img height="144" alt="" hspace="6" src="http://www.plasticsurgerypa.com/blog/Image/wise_pattern_breast_reduction.gif" width="150" align="right"/>Breast reduction (reduction mammaplasty) and breast lift surgery (mastopexy)&amp;nbsp;are similar in that both raise the nipple to the correct position and both reduce the size of the breast envelope.&amp;nbsp; The primary difference between breast reduction and breast lift &amp;nbsp;is that breast tissue is removed during a reduction while none or very little is removed during a mastopexy.&amp;nbsp; </p>
<p>Depending upon the severity of your breast condition, various techniques may be used with different scar configurations. The usual technique for breast reduction is an anchor incision (also known as a Wise pattern -- illustration to the right). </p>

<p><img height="220" alt="" src="/blog/Image/lipoplasty-before.jpg" width="190" align="left"/>Liposuction can be performed for very small volumes of fat, as well as significantly large volumes.&amp;nbsp; Depending upon the patient&amp;rsquo;s goals in terms of body contouring, liposuction can be performed in combination with surgical procedures. Many patients wish to just reduce the overall bulk and would like to look and feel and fit better in their clothing.&amp;nbsp; I have performed liposuction for people who are not at an ideal weight but who afterwards feel much better in that they are fitting better into their clothes and their clothing has actually gone down several sizes.&amp;nbsp; Anything in excess of 5 liters of liposuction is considered large volume and is done in the hospital setting, usually with an overnight stay. Anything less than 5 liters can be done in the outpatient setting with the patient going home that day. </p>

		<![CDATA[<p>It is common for patients to request the simplest and cheapest form of therapy for almost any problem. There are many things that can be done for the aging face including less invasive procedures such as Botox and facial fillers. A wide variety of face lifts gives the surgeon and patients many options. Some of the more common concepts include the so-called &amp;ldquo;threads&amp;rdquo; (of which there are many with different names), mini face lifts, standard face lifts with different treatments of the underlying structures, deep plane face lifts and composite face lifts. A surgeon usually chooses the procedure that gives the best result in his practice and then tailors the procedure to fit the patient&amp;rsquo;s needs. </p>
<p>One of the factors that should be considered when having a face lift is how long the result can be expected to last. American board certified plastic surgeons have spent many years studying the longevity of these various procedures, and have repeatedly debated the pros and cons at our meetings. We all tend to agree that more &amp;ldquo;minor&amp;rdquo; approaches to face lifting operations such as &amp;ldquo;threads&amp;rdquo; and &amp;quot;mini face lifts&amp;quot; do not appear to stand the test of time. Problems with the &amp;ldquo;threads&amp;rdquo; such as breaking, extrusion, and granuloma formation have been reported in our literature. To achieve a lasting facelift most plastic surgeons still favor the traditional facelift. This approach continues to give the patient best result with the fewest complications.</p>

<p>A <strong>traditional facelift incision</strong> often begins in the hairline at the temples, continues around the ear and ends in the lower scalp. Fat may be sculpted or redistributed from the face, jowls and neck, and underlying tissue is repositioned, commonly the deeper layers of the face and the muscles are also lifted. Skin is redraped over the uplifted contours and excess skin is trimmed away. A second incision under the chin may be necessary to further improve an aging neck. Sutures or skin adhesives close the incisions.</p>

		<![CDATA[<p>&amp;nbsp;A patient asked why get injections of Botox if she has to use dilators anyway before attempting intercourse. </p>
<p>Vaginismus is a complicated situation that has a combination of emotional overlay and physical spasm both of which make intercourse either impossible or associated with a great deal of pain and fear. Just because a patient has Botox does not mean that they are cured after the Botox takes effect. </p>
<p><img height="143" alt="" src="/blog/Image/dilators_row.jpg" width="157" align="right"/>Even after the vagina relaxes, there continues to be fear and pain. It is important that the smallest dilator is used for 2-3 days before attempting the next size and so on for about 7-10 days before intercourse. Once you feel ready for intercourse, use the dilator and relax for about 15-30 minutes to allow the vagina to slowly stretch. Lubrication and foreplay are essential, as well as a loving atmosphere. The first several times can be expected to be uncomfortable, but this gets better&amp;nbsp;with time. The photograph to the right represents different size dilators and the handle.</p>

		<![CDATA[<p>Vaginismus is a condition of severe vaginal spasm making intercourse impossible or very painful at the least. Patients who have vaginismus develop an intense fear of any form of vaginal penetration which includes a GYN speculum examination, insertion of tampons, finger or penile penetration.</p>
<p><img height="150" alt="" src="/blog/Image/astroglide.jpg" width="150" align="right"/>For the past several years we have been treating vaginismus with Botox in the group of patients who are resistant to conservative measures or those patients who want to speed up the process of being able to enjoy pain-free sex. This form of therapy is not well known and there appear to be fewer than 50 case reports in the literature.</p>
<p>We have had excellent success using Botox and repeat injections of Botox has not been necessary. One of our patients developed considerable dryness from the injections. This is related to the Botox and is temporary. We suggest that all of our Botox treated patients for vaginismus use water soluble lubricants especially for the first 3-4 months after injection, until the action of the Botox wears off.</p>

		<![CDATA[<p><img height="195" alt="" hspace="3" src="/blog/Image/labia_diagram.jpg" width="200" align="right" vspace="3"/>Labiaplasty is the reduction of excessively large labia minora, the inner lips in a female. These areas project beyond the labia majora, the outer lips of the labia, surrounding the entrance to the vagina. Younger women desire the surgery for either cosmetic reasons, or discomfort with sports such as cycling or spinning classes. Tight jeans may also cause discomfort. Other issues relate to trauma during intercourse.</p>
<p>The older, sometimes menopausal group of women are less frequently seen. They, nevertheless, may experience the same degrees of discomfort, only they begin to notice the onset of these problems at an older age. It comes as a surprise to them because the labia were never a problem in their past. </p>
<p>One of our patients had this type of history. She is a long-term bicyclist and for the past year began having more discomfort in her labia. She also began experiencing&amp;nbsp;discomfort during her daily walks. Examination showed loss of volume involving the outer lips or labia majora. The outer lips padded the area in the past, and with loss of volume, the labia minora were more prone to trauma. </p>
<p>Labiaplasty is the procedure of choice for this condition, and can be expected to improve or cure the problem.</p>
